We are accepting applications from forward-thinking providers at community health centers or rural health clinics who are committed to leading in value-based care and population health, enhancing access, quality, and outcomes for Medicare beneficiaries, and pursuing more predictable, sustainable revenue through capitation payment models.

The enrollment windows for 2026 are June 9-23 and July 15-August 5.

In order to participate starting in 2027, please submit your inquiry no later than July 1.
